AVA Forms for Clubs

It is the desire of the AVA Headquarters to make all files interactive (fillable) as soon as possible. We are currently working diligently on this project. Thank you.

Following is a list of the official forms of the American Volkssport Association. You may reproduce these forms as needed. Each form is dated on the bottom, left-hand corner. Any obsolete forms should be destroyed and only the current ones used. Your cooperation in making sure that these forms are filled out completely, following the directions provided is appreciated.

The forms are in "pdf" format and will require that "Adobe Acrobat Reader" be installed on your computer to use them. If you don't have Adobe Acrobat Reader, you can download it free here. You may open these forms, then print the form.

AVA Form 403 FILLABLE FORM	Certificate of Insurance Application for Additional Insured Used to request an additional insured on a Certificate of Insurance. Cost is $10.00 per certificate if submitted at least 60 days prior to need by date or $15.00 if less than 60 days.	March 2001
AVA Form 403IR FILLABLE FORM	Incident Report Form Used to report accidents that occurred during a sanctioned event.	March 2001

AVA Form 406YS	YRE/Seasonal Quarterly Participation Report Used to report quarterly participation figures at Year Round or Seasonal events. A five part NCR form is mailed with your stamp but you may make copies of this form if you lose the four part one.	November 2002

Title/Instruction forms marked with * are not included in the Policy Handbook. They are either sent at the appropriate time or do not apply to clubs. Some forms that Regional Directors and National Officers use, such as the Event Sanctioning Request Fund Raising Form 402SE are not listed in this area. They can be found in the Executive Council Handbook.
