AVA Forms for Clubs

Following is a list of the official forms of the American Volkssport Association. You may reproduce these forms as needed. Each form is dated on the bottom, left-hand corner. Any obsolete forms should be destroyed and only the current ones used. Your cooperation in making sure that these forms are filled out completely, following the directions provided is appreciated.

These forms are "pdf" forms and will require that "Adobe Acrobat Reader" be installed on your computer to use them. If you don't have Adobe Acrobat Reader, you can download it free here. You may open these forms, then print the form.

AVA Form 201	Application for Club Membership.	February 2002

AVA Form 201U	Club Officer Information Update Update electronically through Club Maintenance site whenever changes occur and annually by November 15	October 2006

AVA Form 102C3	Application for 501(c)(3) Status. Used to request that the AVA file on the club's behalf for IRS Section 501(c)(3) status under the AVA Group Exemption.	March 2001

AVA Form 102C3F Worksheet	Annual Financial Report. Due after 31 July of each year, but not later than November 15.	June 2005

AVA Form 403	Certificate of Insurance Application. Used to request an additional insured on a Certificate of Insurance. Cost is $10.00 per certificate if submitted at least 60 days prior to need by date or $15.00 if less than 60 days.	March 2001

ACORD 25-S(35 kb . pdf)	2008 Certificate of Liability Insurance. The general Certificate of Liability Insurance provided to clubs for their 2008 events. This certificate is not a substitute for those events requiring additional insured, AVA Form 403	January 2008

AVA Form 403IR	Incident Report Form. Used to report accidents that occurred during a sanctioned event.	March 2001

AVA Amateur Athletic Waiver & Release of Liability	Amateur Athletic Waiver & Release of Liability. Used for participants at all events where the waiver is not printed on the start card or log sheet.

AVA Form 405	Event Brochure Checklist. Used by clubs and Region Directors to evaluate each event brochure before it is printed and distributed.	July 2008

AVA Form 406YS	YRE/Seasonal Quarterly Participation Report. Used to report quarterly participation figures at Year Round or Seasonal events. A five part NCR form is mailed with your stamp but you may make copies of this form if you lose the four part one.	November 2002

AVA Form 900A(31 kb pdf)	Event Registration Log showing Free Walkers. Used to register participants at Year Round Events that allow free walkers.	March 2006

AVA Form 907	TAW Advertising Packet. To be used when placing an advertisement in The American Wanderer. Contains rates, sizes, terms and agreement.	March 2001

AVA Form 302 AVA Form 302	Multi-Use Nomination Forms for: Hall of Fame Nomination for Convention. Used to make a nomination for the Hall of Fame. Distinguished Achievement Nomination for Convention. Used to make a nomination for the Distinguished Achievement Award. Meritorious Service Nomination for Convention. Used to make a nomination for the Meritorious Service Award. Certificate of Service Nomination for Convention. Used to make a nomination for a Certificate of Service. Trailblazer Award Nomination. Used to make a nomination for the Trailblazer Award. Ted Ballman Memorial Youth Award Used to make a nomination for the Ted Ballman Memorial Youth Award.	August 2006

AVA Form 302PA (non-fillable pdf)	*President's Award Nomination. Used to make a nomination for the President's Award.	July 1998

Title/Instruction forms marked with * are not included in the Policy Handbook. They are either sent at the appropriate time or do not apply to clubs. Some forms that Regional Directors and National Officers use, such as the Event Sanctioning Request Fund Raising Form 402SE are not listed in this area. They can be found in the Executive Council Handbook.
